  • One (1) Emergency Exam
  • All necessary X-rays (including 1 Cone Beam 3D Scan for in-office use only) valued at $350
  • Additional Benefits of the Plan:
    • 15% off (Fillings, Custom Trays, Sports Guards/Night Guards, In-office Teeth Whitening, Soft Tissue Grafting and Sinus Lifts)
    • 25% off (Porcelain Crown, Fixed Bridges, Dentures, Partial Dentures, Implant Placement, Periodontal Osseous Surgery, Extractions, and Root Canal Treatment)

For Patients with Compromised Periodontal Status

  • Cost: $689 (Valued at over $1500)
  • Four (4) Dental Cleanings per year
